Congratulations on taking the first step in managing your lymphedema by scheduling an appointment with Lymphedema Rehab.  We understand that lymphedema impacts your quality of life, and are glad you have chosen us to help you do something about it.

We also recognize that doing something for the first time can sometimes be intimidating.  So here is everything you need to know about your first visit:

What to bring

  • Your health insurance card
  • A credit card or debit card for your insurance co-pay
  • Patient forms (optional; you can also complete these when you arrive)

When to arrive
Please arrive at least 15 minutes before your scheduled appointment as we do have a few forms you will need to complete upon arrival.  If you would like to complete these forms ahead of time and bring them with you, please click here.

Directions
Lymphedema Rehab is located at 6 Loudon Road (Suite 203) in Concord, NH.  The building is located across the street from the Everett Arena in the five story brick building next to ConvenientMD.

Most GPS navigation systems will take you to the correct location using our address.

From the North:

  • Take I-93 South to exit 14 for NH-9/Loudon Road.
  • At the end of the ramp, take a left onto NH-9 East/Loudon Road.
  • Go over the bridge and take a right at the first set of traffic lights.
  • At the end of the road, take a right. This will take you into the parking lot.

From the South:

  • Take I-93 North to exit 14 for NH-9/Loudon Road.
  • At the end of the ramp, take a right onto NH-9 East/Loudon Road.
  • Go over the bridge and take a right at the first set of traffic lights.
  • At the end of the road, take a right. This will take you into the parking lot.

From the East:

  • Take US-202 West/US-4 West to I-393 West.
  • Take the exit to I-93 South.
  • Take exit 14 for NH-9/Loudon Road.
  • At the end of the ramp, take a left onto NH-9 East/Loudon Road.
  • Go over the bridge and take a right at the first set of traffic lights.
  • At the end of the road, take a right. This will take you into the parking lot.

From the West:

  • Take I-89 South to I-93 North.
  • Take exit 14 for NH-9/Loudon Road.
  • At the end of the ramp, take a right onto NH-9 East/Loudon Road.
  • Go over the bridge and take a right at the first set of traffic lights.
  • At the end of the road, take a right. This will take you into the parking lot.

Parking
There is a large parking lot next to the building with ample space for parking.  From the parking lot, follow the building’s signage to the main entrance.

Finding our office
From the main entrance, take the elevator to the second floor.  Take a right out of the elevator.  Lymphedema Rehab will be located down the hall on your left (Suite 203).

What to do when you arrive in the waiting room?
If we are with another patient when you arrive, please have a seat in our waiting room.  We will be with you as soon as we can.

Payment/Insurance
As part of the check-in process, the therapist will take your completed paperwork, scan your insurance card, and process your co-payment.

Evaluation
Your first appointment is for our therapist to get a baseline on your medical history and an understanding of how lymphedema is impacting your life.  In addition, the therapist will explain our treatment process, discuss how the treatment will help make a difference, and also take a few measurements.

That’s it!  Before you know it, you’ll be on your way with a plan in place to manage your lymphedema.  The average plan of care calls for the patient to be seen for 6-12 visits (depending upon your level of need), but can be tailored to fit the patient’s needs and availability.
